We Welcome New Patients
During your new patient consultation, we enjoy taking the time to address your chief concerns, and most importantly, establish a meaningful doctor patient relationship that will last for years to come. If you are looking to become an established patient of the Mattson Hellickson Dental, follow the steps below:
- Step 1: Please click here to get started, by completing our new patient forms and enter any arbitrary date to access the forms (this will not be your appointment date).
- Step 2: Once both forms are complete and submitted electronically, give our office a call to schedule your initial appointment.
- Step 3: Please reach out to your previous dental office to have dental records sent to newpatients@mh.dental – it is your responsibility to ensure these records have been sent.
What to Expect
We hope this will be the first of many positive experiences you have with our office. Your initial comprehensive exam is an opportunity to get to know you better, while addressing your primary concerns.
A comprehensive exam not only consists of an examination of all of your teeth, but also the associated supporting structures inside your mouth. A full series of x-rays will be required and taken at this appointment, unless you have a full mouth series of x-rays or panoramic image taken within the past 5 years. These must be sent to our office BEFORE your initial visit. The information collected during your first visit will enable our doctors to create a personalized treatment plan tailored to your needs.
Mattson Hellickson Dental is preferred provider with Delta Dental Premier. All other dental insurance plans are considered out of network. We do accept out of network dental plans and we will do everything we can to ensure your insurance plan is utilized to maximize your benefits.
